Atypically sited lesions in recurrent HSV-1 infection


In individuals with a diagnosis of genital herpes, recurrences can occur anywhere in the dermatomal area of the initial infection. For instance, in this patient who had initially experienced a vulval and cervical primary episode due to HSV-1, the appearance of lesions on her left buttock 3 months after the initial episode was most alarming. The patient did not link the two and was concerned that she had acquired a new infection. The major differential diagnosis is from varicella zoster and, although HSV-1 and varicella zoster are both treated with oral antiviral drugs it is important that they are differentially diagnosed. The best way to do this is by virus sampling, either by culture or PCR, from the lesions. In cases of diagnostic confusion, serological tests can be helpful. The patient must be told that these lesions are infectious, in the same way that lesions on the genitalia are infectious.
