Paraphimosis in primary genital herpes

Primary genital herpes can be accompanied by local complications such as the case shown here where a paraphimosis developed secondary to penile oedema due to primary HSV infection, and also as a result of the patient’s self-treatment with a concentrated topical antiseptic. Treatment comprised the prompt commencement of antiviral treatment, in this case using aciclovir 200 mg five times daily, admission as an inpatient, administration of a potent analgesic (in this case intramuscular pethidine), and an attempt to replace the foreskin over the glans in a warm bath. Fortunately, the attempt was successful and surgical intervention (circumcision) was deemed unnecessary.
