Daily antiviral medication should be the preferred choice for managing most patients with genital herpes simplex virus (HSV)-2 infection. Suppressive therapy offers several medical benefits over episodic therapy. These benefits include reduction in frequency of recurrent outbreaks, reduction in the frequency of viral shedding, reduction of transmission of HSV to sexual partners, and a probable reduction in susceptibility to HIV infection. In addition to the medical benefits of suppressive therapy, patients have improved psychological status when outbreaks are occurring less often. For patients with new infection, suppression may be particularly helpful as these patients shed more virus, have more outbreaks, and are working on emotionally adjusting to having a lifelong sexually transmitted infection. Generic aciclovir is within the price range of most patients. Suppressive therapy should be offered to all patients, and strongly recommended to many, depending upon their social and sexual circumstances.
